Main Admitting
Located at 1900 Sullivan Avenue, Room 1120, 1^st floor, just off the main entrance to the Hospital.

  6 a.m.-7 a.m., Monday-Friday

• All Other Registrations
  7 a.m.-5:30p.m., Everyday
  Closed on all holidays.

Emergency Room Registration
Located at 1900 Sullivan Avenue, on the west side of the main entrance to the Hospital.
Always open.

Outpatient Registration (STAR)
Located at 1850 Sullivan Avenue, Room 120, on the 1st Floor of the Hospital.
7 a.m.-5:30 p.m., Monday-Friday
Closed on all holidays.

Ambulatory Surgery Center (ASC) Registration
Located at 1850 Sullivan Avenue, on the 4th Floor of the Hospital.
8:30 a.m. to 5 p.m., Monday-Friday
Closed on all holidays.

SpineCare Registration
Located at 1850 Sullivan Avenue, on the 2nd Floor of the Hospital.
8:00 a.m. to 5:00 p.m., Monday-Friday
Closed on all holidays.

CDI-Campus Diagnostic Imaging
Located at 901 Campus Drive in Daly City.
9 a.m.-5:30 p.m., Monday-Friday
Closed on all holidays.
